Report by Phone:  (855) 650-0005

The Town of Fairfield is committed to maintaining a high level of integrity and transparency in all areas of Town government. To that end, the Town has contracted with Lighthouse Services, Inc., to provide an anonymous and confidential reporting service for potential fraud and/or ethical issues. Reports may be submitted 24 hours a day using a secure online form or via a toll-free number: (855) 650-0005.

Please use this service for ethical or fraud-related concerns only. To report any other type of concern or to make a request for service, please use our Citizen Service Request System.

How It Works

Residents, employees, volunteers, board or commission members and other members of the public can report potential fraud incidents and/or ethical issues using an online form or the toll-free number, (855) 650-0005.  Lighthouse Services then automatically routes the incident report to the Human Resources Department and all members of the Ethics Commission

Reports can be submitted anonymously or the submitter's contact information can be provided. The Town will attempt to limit the disclosure of any information provided as much as possible. Lighthouse will not disclose a submitter's identity without express permission.  However, it is possible that the submitter's identity and/or the substance of the incident report may be disclosed by the HR Department and/or the Ethics Commission to third parties during an investigation of the matter because of the information provided. In addition, the Town may be required to disclose the submitter's identity and/or the substance of the incident report in response to a request made under the Connecticut Freedom of Information Act, a lawfully issued subpoena, or a court order.

The Town of Fairfield will not retaliate against anyone for submitting an incident report. This includes, but is not limited to, protection from retaliation in the form of an adverse employment action.  Any submitter of an incident report who believes he or she is being retaliated against should immediately contact the Human Resources Department.
